- Applicable to Parents/Guardians of minor children.
Instructions:
- Carefully review the above-named documents;
- Parents/Guardians of minor children must complete the Per Capita Distribution (PCD) Release and Indemnity Form in highlighted areas;
- Photo copies, faxes, texts, or scanned email copies of PCD Release and Indemnity Forms will not be accepted;
- The original completed form must be returned by one of the following:
- In person drop off at the Administration Building (On Reserve)
- In person drop off at the ADC/LPFN Finance (Misk)

Original completed PCD Release and Indemnity Forms must be returned by Friday, November 12, 2021.
Original completed PCD Release and Indemnity forms received after this date will be processed in the
new year (2022).
PER CAPITA DISTRIBUTION (PCD) – RELEASE REQUIREMENTS & REQUIRED DOCUMENTATION
Required documentation – Tribal members pick up in person or mailing for Minor(s)
- Confirmation member(s) name is on band list;
- Completed original Release and Indemnity for adult accepting minor’s payment;
- A letter of authorization must be obtained from the parent(s) if you are claiming minor’s funds and you are not the parent;
- A letter signed by both parents stating who is to receive the minor’s funds must be submitted with a copy of a valid Photo ID, if you are under a custody agreement.
*ALL MINORS’ PAYMENTS WILL BE WITHHELD UNTIL FULL REQUIRED DOCUMENTATION IS PROVIDED*
Required documentation – Incapable of managing his/her own affairs – Pick up in person or mailing
- Confirmation member(s) name is on band list;
- A notarized copy of the power of attorney and/or the guardian of property of such individual appointed by a court or Indigenous Services Canada (ISC);
- Completed original Release and Indemnity (signed by the attorney or guardian named – in that capacity for a minor); and
- One piece of valid ID.
Note: Notarized copy of the power of attorney – Proof of mental infirmity by a doctor will be required if the power of attorney is contingent on a mental infirmity.
Required documentation – Deceased After August 1, 2021 – Pick up in person or mailing by Administrator of Deceased Estate
- Confirmation member’s name was on band list on August 1, 2021;
- A notarized copy of a court appointment or ISC appointment, as to the Administrator of the deceased’s estate;
- Copy of Death Certificate.
