Long Plain First Nation is seeking two (2) full-time Off Reserve Case Manager’s for Jordan’s Principle

Under the direction and supervision of the Jordan’s Principle Program Manager, the incumbent will be responsible for providing support to Long Plain First Nation (LPFN) members requiring health and social services. The Jordan’s Principle Case Manger ensure children and families receive services without denials, delays or disruption of services due to jurisdictional disputes.

DUTIES & RESPONSIBLITIES:

  • Assist with coordinating comprehensive client services.
  • Assess the deficits with service systems and the need to protect children through early intervention, sharing of information, effective coordination of services and appropriate training.
  • Collaborate, network, and cultivate partnerships to ensure a comprehensive referral network for community resources.
  • Follow service coordination and workflow process from intake, ongoing case management and discharge.
  • Work in collaboration with assigned finance department staff to ensure that all financial claims related to Jordan’s Principle are processed and managed according to policy and procedures.
  • Liaise and work effectively with the community, service providers, collateral agencies, organizations, knowledge keeper and elders.
  • Formalize monthly reports in collaboration with the Program Manger that report Jordan’s Principle service coordination service review, analysis and referral activity and service gaps related to mandated and non-mandated service areas.

QUALIFICATIONS:

  • BN/RN, RN, LPN, Social Worker, Physiotherapist, Occupational Therapist, Teacher in good standing with their professional licensing bodies.
  • Minimum two (2) years of experience working in the field of health, social or education sectors.
  • Minimum one (1) year direct service experience with children and families.
  • Knowledge of the Jordan’s Principle initiative.
  • Knowledge of health care system along with the Indigenous Service Canada system and their respective roles.
  • Knowledge of culture, traditions, attitudes, and socio-economic conditions if the First Nations communities.
  • Knowledge of LPFN programs and services.
  • Must be highly organized, punctual, and reliable.
  • Proficient in the use of Microsoft Office Applications.
  • Must have excellent written and verbal communications skills.
  • Attention to detail and strong interpersonal skills.
  • Strong ability to independently solve problems.
  • Ability to establish priorities, work independently, and proceed with objectives without supervision.
  • Knowledge of population health and First Nations perspectives on health and Wellness.
  • In-Depth knowledge of First Nations health and wellness system needs.
  • Knowledge of relevant legislation such as the CFS Act and Vulnerable Persons Legislation.
  • Good organizational, time management and prioritizing skills.
  • Ability to function effectively as a team member.
  • Must be able to travel as requested.
  • Be able to work a flexible or irregular work schedule including afterhours and weekends, as needed.

CONDITIONS OF EMPLOYMENT

  • Valid Manitoba Class 5 Driver’s License and access to a reliable vehicle.
  • Valid CPR and First Aide Certificate
  • Clear Child Abuse Registry Check; Adult Abuse Registry Check; Criminal Record Check with Vulnerable Sector Screening
