Long Plain First Nation is seeking a full-time Community Health Nurse
As part of the Long Plain Health care team, the incumbent delivers comprehensive health promotion, community health and disease prevention programs for health concerns identified by the community. These programs are designed in partnership with community members and leaders and help to create environments that support good health practices for individuals and communities. Community health nursing programs include health promotion, prevention, health protection, education and utilizes population health approaches in planning, implementing, and evaluating programs. DUTIES & RESPONSIBILITIES:
- Assess, develops, delivers, and evaluates, using models of health promotion and best evidence, Community Health Nursing programs in partnership with community participants, making recommendations on program priorities and needed resources.
- Uses culturally sensitive community development strategies to partner with community members to identify and address their shared health concerns. Build on existing community strengths to maintain and enhance good health practices
- Advocate for individual and systems wide access to needed programs to deal with health issues. Promote health public policy.
- Collaborates with individuals, families, groups, and community to identify potential health risks and make choices for appropriate health management strategies (i.e., health surveillance programs, communicable disease control, immunization, and screening for disease.
- Establish intersectoral partnerships which involve individuals and communities in collaborative relationships with health care professionals, community organizations, volunteer service organizations etc. to help address broad determinants of health (income, social status, education, employment, and work conditions).
- Collaborate with community partners to address disaster planning and emergency preparedness.
- Purses professional development relevant to community nursing practice and utilizes self-reflection practice to evaluate own care.
- Contributes to making the organization safe for clients and staff and recognizes the importance of reporting unsafe situations and participates in follow up reviews as a learning opportunity.

QUALIFICATIONS:
- Baccalaureate Nursing Degree is required.
- Active practicing member on the RN extended practice (ep) register of the College of Registered Nurses of Manitoba (CRNM).
- Basic life support (bls) certificate.
- Two (2) years in critical nursing or community health screening.
- Knowledge of the College of Registered Nurses of Manitoba Standards of Practice for Register Nurses, Canadian Nurses Association Code of Ethics for Registered Nurses, and Regulated Health Professions Act.
- Advanced nursing care knowledge related to health assessments, diagnostics, and referrals.
- Knowledge of the Personal Health Information Act (PHIA), Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions) and other relevant legislations acts.
- Demonstrated ability to effectively manage workload by establishing priorities.
- Demonstrated knowledge of customer service concepts and practices
- Excellent interpersonal skills with the ability to demonstrate collaboration, encouragement, and a patient approach.
- Experience working within First Nations communities.
- Knowledge of First Nations culture, history, and issues.
- Demonstrated superior written and oral communication skills.
- Experience utilizing a variety of computer applications, such as Microsoft Word, Outlook and electronic medical records charting systems.
- Demonstrated ability to perform duties in a confidential and ethical manner.
- Ability to function effectively as a team member, demonstrating emotional stability and objectivity when dealing with challenging/changing work environments.
- Excellent interpersonal skills and ability to work co-operatively as a part of an interdisciplinary team of professionals.
- Demonstrated ability to work independently with minimal supervision.
- Knowledge of Indigenous culture, traditions and fluent in a First Nation language would be an asset.
- Adhere to LPFN policies and procedures with specific attention to attendance, and professional ethics.
- Must be in good physical and mental health.

- Valid Manitoba Class Driver’s License and access to have a reliable means of transportation.
- Must have a Criminal Record Check, Clear Child abuse Check.
- Valid CPR and First Aide Certificate.
