Moderna & Pfizer VACCINATIONS – 1^st dose, 2^nd Dose & Booster shots

Pediatric Pfizer Vaccinations 5-11Yrs

vaccination mass clinic will be held at the Long Plain Rez Plex:

• Wednesday December 29^th 9:30am – 2:15pm
• Thursday December 30^th 9:30am – 2:15pm

PLEASE NOTE:

• ALL CHILDREN MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN
• Transportation is available for on-reserve clients, please notify when booking appointment.
• Appointments preferred; Walk-in’s will be taken.
• Doses will not be held for missed appointments.
• Clients are responsible for booking their follow-up appointments, i.e., 2^nd dose.
• These clinics are for Pediatric Pfizer Vaccinations, Children 12+ and Adults 18+
• Bring health card and proof of immunization if first dose was not received through Long Plain Health Center.

Eligibility Requirements:

All Long Plain First Nation registered members on-reserve and off-reserve along with their immediate family household

Any other Registered First Nations and Non-First Nation population who reside or work DIRECTLY in Long Plain First Nation
