POSITION: Home and Community Care Nurse

Under the direction of the Home and Community Care Program Coordinator, the Licensed Practical Nurse (LPN) is responsible for providing community based acute care nursing services. Using their teaching, assessment and nursing skills to effectively assist clients to attain their optimum level of independent functioning.
Key Responsibilities
- Provide in-home and in-facility home care services as scheduled by the Home and Community Care
- Effectively organize home care nursing services within the home in an innovative and adaptable
- Maintain proficient client records and statistical monthly reports documenting services
- Provide support to the client and their families by acting as a resource as well as a professional liaison.
- Maintain current nursing skills through attendance at workshops, in-service sessions, reviewing appropriate
- Function as an active member of the Home Care Team and network with other community workers as needed to provide adequate community health promotion activities.
- Provide acute nursing care in home or in-facility based on physician instructions and as outlined in the Home Care and Community Care Service Delivery plan.
- Consult with client’s physicians or other health care professional as necessary to provide coordinated client
- Provide information and teachings to client’s care givers on self-care, prevention, rehabilitative and comfort
- Provide personal care to client’s whose condition is so unstable that it cannot be safely carried out the Health Care Aide or family
- Other duties as assigned and/or outlined in the job
Qualifications
- Must possesses a current active registration as a Licensed Practical Nurse and be a member in good standing with the College of Licensed Practical Nurses of
- Previous Home Care or Community Health work experience will be an
- Must have the ability to perform all direct care nursing tasks as legislated under applicable professional nursing
- Excellent interpersonal skills with the ability to demonstrate collaboration, encouragement and a patience
- Strong organizational skills with the ability to prioritize tasks with competing
- Experience working within First Nations
- Knowledge of First Nations culture, history, and
- Demonstrated superior written and oral communication
- Demonstrated ability to perform duties in a confidential and ethical
- Ability to function effectively as a team member, demonstrating emotional stability and objectivity when dealing with challenging/changing work
- Excellent interpersonal skills and ability to work co-operatively as a part of an interdisciplinary team of
- Demonstrated ability to work independently with minimal
- Must be proficient in variety of computer software and applications such as electronic
- Knowledge of Indigenous culture, traditions and fluent in a First Nations language would be an
- Adhere to LPFN policies and procedures with specific attention to attendance, and professional
- Be able to work a flexible or irregular work schedule including afterhours and weekends, as
- Must be able to travel as

Conditions of Employment
- Valid Manitoba Class 5 Driver’s License and access to a reliable Valid CPR and First Aide Certificate. Satisfactory Criminal Record Check, Clear Child Abuse Checks.
