Long Plain First Nation is seeking a full-time On-Reserve Case Manager for Jordan’s Principle

Under the direction and supervision of the Jordan’s Principle Coordinator, the case manager will be responsible to provide assessment, follow up and will liaise with all other outside agencies for services. The Case Manager will provide support to children with complex needs and their families. Services provided include child development, respite care, crisis intervention, behavior management, and life skills, whether on a one-to-one basis or in a group environment. A key component of this position is to assist and support in the clients’ physical, social, emotional and daily life skills development based on the seven domains of development.

DUTIES & RESPONSIBILITIES:

  • Provide up to date client status information to the Program Coordinator.
  • Develops and documents a coordinated service plan that includes referral, assessments, treatment/admission, discharge (if applicable), transition and ongoing care protocols.
  • Communicates the service plan to children and family and appreciate organizations.
  • Ensures the appropriate and effective sharing of information including identifying and maintaining contact with children and families as required for continuity of care.
  • Provides navigation services with a cultural safety and humility lens, using a trauma-informed and culturally competent approach. Understands the importance of working with community members in order to make children and families comfortable with their interaction with the health care system.
  • Builds strong relationships and partnerships with the front-line health care and childcare workers, and works with a variety of health care and child care professional, including but not limited to nurses, speech language pathologists, dental, occupational therapists.
  • Facilitates access to primary care and advocates for children and families when they identify service delivery gaps or issues.
  • Proactively builds front line and community relationships to bring children and families with complex care needs to the right organizations and expedite assessments and referrals with all areas of the health care system.
  • Develops a strategy that ensures children are fulfilling their life-time potential.
  • Provided community-based services like workshops, presentations, and team building activities focused on developing.
  • Ensure a model of service delivery for children with complex needs and their families.
  • Takes the lead in securing effective development, delivery, and review of services for clients.
  • The Case Manager and Child Development Workers will strive to create a fun and enjoyable atmosphere to encourage client participation in programs.
  • Works in coloration with Manitoba Regional Health Authorities and Children’s Rehab services to ensure client’s needs are met while work is underway to create efficiencies, shared training, assessments methodology and tools.
  • Emphasizes health promotion and illness and injury prevention and provides appropriate anticipatory guidance.
  • Provides formal presentations and education programs as well as informal teaching to families, groups, individuals, and the community.
  • Uses clinical skills to assess the client’s health status to enable joint planning, implementation, and evaluation of appropriate interventions.
  • Uses appropriate health promotion, illness, and injury prevention techniques.
  • Promotes and supports patient safety by addressing client safety issues and concerns as they arise.
  • Coordinates appropriate care and equipment including case management with other community.
  • Liaisons with family, medical and other resources, as required.
  • Other duties as assigned – assist in all other functions/duties deemed necessary by the Jordan’s Principle Coordinator. The Jordan’s Principle Coordinator may add duties to assist other program staff, as required.

EDUCATION, SKILLS AND WORKING CONDITIONS

  • RN/BN, RN, LPN, or Social Worker – with current active registration. Teacher or BA in Social Services, Health, Education.
  • Grade 12 or equivalent.
  • Minimum three (3) years of experience working in the field of health, social or education sectors.
  • Must be highly organized, punctual, and reliable.
  • Attention to detail and strong interpersonal skills.
  • Excellent written and verbal communications skills.
  • Requires a current Child Abuse Registry Check; Adult Registry Check; Criminal Record Check with Vulnerable Sector Screening.
  • CPR & First Aide Certification or willing to obtain.
  • Valid driver’s license.
